What to bring

We highly recommend visiting the Patient Portal to input medical information directly into our Electronic Medical Records (EMR) system. This will save time on the day of the appointment and allow you to accurately enter your health information in a comfortable and relaxed setting. You must be a registered patient to sign up for the patient portal. It is required to provide your name, date-of-birth, email address, and a minimum of the last four digits of your social security number during registration to ensure portal access.

If you choose not to fill out the necessary information via the patient portal, please arrive 30 minutes prior to your appointment to complete the necessary information. If you complete all necessary patient portal information, please arrive 10-15 minutes early to provide the documents listed below.

Approximately 5 days prior to your appointment, if you provided an email address during registration, you will receive an email with information and a link to register for the portal. Please utilize the link in the email, or the website link, to complete or update your medical information.

1) Driver’s License or Photo I.D.
2) Insurance Card or Cards
3) Medication List (include your dose and how many times a day it is taken)
4) Any X-rays from your referring physician/urgent care/hospital
