Our Policies:

Blue Cross Blue Shield (BCBS): Our physicians have contracted with most BCBS plans (Traditional, Preferred Patient Care, Network Blue, Advantage 65).  We will collect any co-payment or deductible required by your plan at the time of service and file your claim.  Patients are expected to pay their portion as determined by BCBS.

Medicare Part B: Our physicians are participating with this insurance plan.  We collect the 20% co-insurance at time of service unless a patient has a secondary insurance that pays the full amount directly to us.  Non-covered items are paid as incurred.

Other Medicare plans: We will file these insurance plans as a courtesy and will accept Medicare Part B allowable rates.  Patients are responsible for any co-pays and deductibles at the time of service.  Any difference between what your insurance pays is your responsibility.

Other Commercial insurance:  We request payment in full unless you have out of network benefits.  Then, we would collect applicable co-pays and deductibles.  We can file as a courtesy or give you the necessary paperwork to do so.

Automobile:  We collect at time of service and will file as a courtesy or give you the necessary paperwork to submit to your insurance carrier.

No Insurance (Self Pay): Patients are expected to pay in full at time of service.  Ask our office about the set rates for new patient problems.

Non U.S. Citizens:  Patients are expected to pay in full at time of service.  We may be able to file your insurance as a courtesy.

We accept cash, check, VISA, MasterCard and Discover.  You can pay in person at the office, via mail, or call and provide the credit card information over the telephone.
