FIRST VISIT

We are currently accepting new patients.
  • What to Bring to your Visit:
  • A valid driver's license or ID card
  • Your medical insurance card
  • Payment method: cash, check, or CC
  • ALL x-rays, scans and tests relevant to your visit
  • List of medications and medical history form
  • A referral letter (if your insurance requires one for claims)
  • To help ensure a smooth first visit, please complete your intake form through the Patient Portal prior to your appointment, or arrive 30 minutes early to allow time for registration.
CO-PAYMENTS AND DEDUCTIBLES
All co-payments, co-insurance, and deductibles must be paid at the time of service and will be collected upon check-in. This is a required contractual obligation with insurance companies.
PRESCRIPTIONS & REFILLS
For prescription refills, please allow 48 hours for medications. Requests for medication refills will not be processed outside business hours.
EMERGENCY CARE & AFTER HOURS

In an emergency, call 911 or go to the nearest emergency room.

If you are at a local hospital or urgent care center listed below and need orthopaedic care, you may request one of our physicians. Kindly ask the emergency room staff to notify our office at (941) 497-2663.

Established patients with urgent orthopaedic concerns may call our office after hours and leave a message with the answering service; a physician will return your call.

For billing or insurance questions, please call during regular business hours.
